added NFe, MDFe and CTe servers and scrapers

extracted and cleaned from #121

This is the list of servers and endpoints for NFe, MDFe and CTe. It's generated with a small scrapers using pandas an Beautifulsoup. There are development dependencies, not new runtime dependencies or course.

The only files containing real code are in nfelib/utils, the other files, the list of servers is generated and the tests are testing that the list is the same as before or it will break into an error so we detect the changes.
