Add options for accumulating all footnotes & citations into lists

Add two command line flags (one for footnotes, one for citations) for controlling whether to extract all footnotes & citations from the body and store them in the fields content.footnotes and content.citations. Each field should be a list of objects with the fields label, body, ids, backrefs, names(?), and (footnotes only) auto(?).
