As added by the eMediplan ePrescription rev3, we will also need to explicitly add support for this at the MedicationRequest and MedicationDispense level. Possibly at the administration level as well. See CARA-ch/ch-emed-epr#103 for more details. I believe since this is not anyway a mandatory element and having it in the core profiles could be of general use, this could be done at CH Core level.

One of the candidates at the MedicationRequest level could be for instance reasonCode (without excluding any other potential candidates), but we would request the FOPH to at least provide a naming system for such codes so that they can be differentiated from other codes.

Perhaps the best solution that could be used for the three directly affected resources (med. request, dispense and admin.) in a coherent way would be an extension after all. In this case, I'm guessing the best course of action would be to ask the FOPH to add such an extension directly in the EPL IG so that we can then use it in the CH Core profiles. This would require adding EPL as a dependency to CH Core, which I do not know if it is something we'd like to avoid.
