LU_DATABASE file format

Has anyone recently used the anthropogenic disturbance option (IANTH_DISTURB = 1) in ED2? I was wondering if I could use it to simulate slash and burn agriculture but I'm having a hard time finding any documentation about it ... In particular, I was struggling with the format of the LU_DATABASE file.

I have found somewhere in my archives an example file but was unsure it was up-to-date ... Is there a recent example (ideally with description) available somewhere? Mine looks like this:
